Prepare a medical exhibits folder for each consultative examiner as follows:
Use a letter-size folder. On the front of the folder, write or type the claimant's name and Social Security number, and the type of CE requested.
Make legible copies of the material and relevant evidence identified by the ALJ as related to the type of examination ordered along with the most recently completed disability report form: i.e. the SSA-3441 (Disability Report-Appeals). Material that is not relevant to the type of examination ordered should not be included.
